Take-‐Home Naloxone to Be Prescribed Primary care clinics will be prescribing take-‐home naloxone for patients at risk for opioid overdose, this includes patients on long term opioids for pain and those abusing prescription opioids or heroin. Formulations: naloxone can be given by intramuscular (IM) injection or intranasal (IN) administration. Most patients will be prescribed IN naloxone because it is equally effective and more convenient. Dispensing: Intramuscular Intranasal Naloxone 0.4mg/ml single dose vial, 2 vials NDC # 00409-1215-01 SIG: Inject 1 ml IM upon signs of opioid overdose. Call 911. May repeat x 1. Syringe 3ml 22G X 1 ½ " # 2 SIG: Use as directed for naloxone administration. Naloxone 2mg/2ml prefilled syringe, 2 syringes NDC # 76329-3369-01 SIG: Spray one-half of syringe into each nostril upon signs of opioid overdose. Call 911. May repeat x 1. (Atomizers will be given to patients in clinic. Please verify the patient has received the atomizer before picking up prescription.) Patient Counseling: naloxone is a bystander administered drug which means family and caregivers should be included in patient counseling. If they are not available then the patient should be instructed to train others in case of an overdose. See attached patient counseling checklist for IM and IN formulations. Billing and Formulary Status: Naloxone vials and prefilled syringes are covered by all local health plans including Healthy San Francisco, San Francisco Mental Health Plan, and MediCal. For the MediCal HMO plans naloxone is a " carve-‐out " medication. This means that claims will have to be submitted directly to FFS MediCal. Do not send PA requests to the MediCal HMO health plans. Most Medicare plans also cover both formulations of naloxone.
